For our records, new clients must fill out a new client form. We give new clients a few options, so do what is most convenient for you! You may complete this form before your pet's first visit, but please arrive a few minutes early to do so. You may also print and complete this form at home at your own convenience, and fax it to us at 309-647-9525 or simply bring it to your pet's scheduled appointment.

The first time you visit us, we need to get a clear picture of your pet's complete health. To do this, one of our doctors will perform a nose-to-tail wellness exam, checking everything from their teeth and gums, to their joints and muscles, and everything in between. Based on your pet's unique medical history, we may also recommend additional services such as vaccines, lab work and parasite testing. You can also expect us to discuss various aspects of your pet's health such as dental care, parasite control, spay/neuter procedure, microchips, diagnostic testing, nutrition & activity level and behavior & training.
